Dynamic Skill GUI form to enter schematic device

Hi 

I want to create a GUI with the following requirements similar to library path editor. It is my first time to create dynamically adjusted form. I am not sure that it is doable or not.

Do you have sample code or any document to show how to create dynamic form described in step 1-3 ? Any suggestion is welcomed.

1. Click button "Add constraint for devices" in the form and an entry is created.  I enter a few device names and constraints in the entry. The entry contains string line and a few radio buttons.

2. Click button "Add constraints for devices" in the form again and another entry is created.  I enter a few device names and constraints in the entry..

3. Repeat step 1 until I enter all devices.
