input data parameters problem

disclaimer:this is done in pspice 10.5 which is old but i cant use another version because my university obligates us to use it.

hello,

how do i put the input parameters from the data sheet ?

i finished the design in capture cis but when simulating this error show up i don't know how to declare those parameters.
