Adding Intersheet References with Hierarchical Ports or Design

I'm having trouble getting intersheet references to annotate on my design. I have set the schematic up using a hierarchical design and have connected everything in a top-level block diagram. 

I'm going to Tools > Annotate and select "Add Intersheet References". Is there something I am missing?

    Probable reasons of intersheet references not working on schematic could be - 

    • There are more than one title blocks per page.
    • There is no title block for a page.
    • The page number and page count property values on the title blocks are incorrect.
    • Your design has duplicate page numbering.
