Square box with X in it

Some of the traces on a board I'm working on have a square with an X in it. Could be at the vertex of a trace or on a pad. What does this mean and how do I get rid of it?

    This is unusual, but now every GND pad has one of theses "X in a box" symbols instead of a ratsnest. I've always had rats showing - been doing this for more years then I care to remember and now boxes with X's showing up.............?

    Ok, so some investigation. In the constraints under Net / General Properties, the GND net has 0V next to it. If I remove that the rats nest comes back.

    If I reload the netlist it reverts back so it is coming from Capture - just where I'm not sure............ yet
