I recently released a BOM where the Reference designator generated by the BOM does not reflect the .dsn file or the .brd file. Is this a bug? I re-ran the BOM to verify and indeed it is being changed.
Something wrong with your output process. There is always the instance vs occurrence view. You probably are mixing them?
You might need to run the accessories tool to update one to the other...
