Dynamically adding or deleting the fields in a GUI

HI,

I would like to create a GUI where I could add some fiellds or delete some fields after popping up the GUI.

Best example to describe the problem is like when we open Find/Replace form in layout editor using (Shift+s)  bind key.

We are allowed to create some new fields by clicking "Add Criteria" and we could delete a field by using "delete" buttom.

I would like to create  a GUI similar to this. I have attached a snap of the GUI.

Can anyone please post a sample code for it.
