How do you annotate region of operation for NMOS transistors in Cadence Virtuoso?

Virtuoso version: IC6.1.6.500.13.2

I am trying to: print region of operation for all NMOS transistors on my circuit schematic.

Pictures attached: circuit schematic, annotation setup

Here's what I have so far:

1) View -> Annotations -> Setup

2) Annotation Settings for: Library (analogLib), Cell (nmos)

3) Clicked on a green plus on top left corner to add a new Label

4) Changed Display Mode of newly created label to "Component Parameter"

5) Changed Expression to "region"

From here on out, all I thought I'd have to do is check the "Annotate" box and then I'd be good to go. However, I cannot check the box. It won't let me. Does anyone understand the problem?

    The number of parameters that can be annotated is governed by the number of cdsParam(N) labels on the symbol itself. It looks in this case as if there are 3 - which is why the fourth shows up in a different colour on the form. That depends on how the symbols are defined in your process design kit.

    If you go to the column headings on the annotation->setup form, you can do Right Mouse->Balloon Column (or you can use File->Show Balloons which would also cause the column to appear). Any of the annotations beyond the maximum number of labels can have the Balloon column checked - and if balloons are being shown (which is what File->Show Balloons does), then hovering over the transistor on the schematic would show a balloon with the information you want.
