Error found by spectre during hierarchy flattening.ERROR(CMI-2116): I91: Too few terminals given(4<6).

When I run simulation using spectre, it shows the error in the title. I91 is a 3-input adder written by veriloga. 3 input + 1output, It should be 4 terminals, but it says(4<6). I checked every sub-cell(schematic), there is no warning or error. I can't figure out what's wrong with it?

    Luckily I have solved this problem, it turns out that the module name in add_5 and add_3 are same, so add_5 override add_3.
