output format for form display.

Hello,

I have a piece of code below for which I would like to get a formatted output like below. Is it possible to display below output format in hiCreateAppForm?

    What are you asking here? When you run your code, you get:

    Apart from this being a bit odd, it's not clear to me what it is you wish to get that is different from the above picture?

    Like below, I would like to have a check box for each line and display format should align with a tabular form. Thanks for your help here!

    If you only want a single checkbox per row, I would put a toggle field to the left of a report field (or four boolean buttons left of a report field).
