output format for form display.

Hello,

I have a piece of code below for which I would like to get a formatted output like below. Is it possible to display below output format in hiCreateAppForm?

    If you only want a single checkbox per row, I would put a toggle field to the left of a report field (or four boolean buttons left of a report field).
