Module coverage lower than instance coverage

My coverage results as viewed in the ICCR GUI show cumulative block & expression coverage at 70% for Instance, but only 1% for Module/Unit.  What could cause this?  It seems that Module/Unit coverage should always be greater than or equal to Instance coverage.
