DRC reporting phantom Unconnected Pin errors

I have am in the process of finishing up my routing so I am using the Unconnected Pin report to find my missing connections.  Unfortunately, the first 3 ones I have checked are all phantom, the connection is there and the report is incorrect.

Is there any special configuration for this report?  Is there something like the trace has go to the center of the pin?  I am confused because the ones I have checked are obviously connected.

The report is always correct. You need to ensure that you connect to the centre of the pad to make the connection, alternatively you could get close then RMB finish.
