i cant't create a package symbol

 hi, i'm tryin to create a new package symbol but at the end of the process i get this error:

¿what am i doing wrong?

    i did it just like you told me but i don't understand what you mean with "add "*" at origin"  and " Add refdes to both ASSEMBLY_TOP & SILKSCREEN_TOP" because as you can see in the image (in the link) theres no texbox to write anything, when you click layout->labels->refdes.
